寰椎（Atlas），又稱第一頸椎，英語簡稱C1，是頸椎的第一節。寰椎英文名以希臘神話中背負地球的泰坦巨神阿特拉斯的名字命名，因為它支托人類頭部的全部頭骨[1][2][3]。

寰椎外形呈環形，無椎體結構，其椎體已經與下方的樞椎融合[4][5][3]。

寰椎與樞椎構成連接頭骨與脊椎的寰樞關節，與枕骨構成寰枕關節（英語：Atlanto-occipital joint）。點頭的動作主要通過寰枕關節的屈伸發生，轉動頭部的動作則主要依賴於寰樞關節[5][6][7]。

由於腦幹向下延伸到樞椎，致使寰椎和樞椎在神經學研究上極為重要[8][9]。

結構[編輯]

前弓[編輯]

前弓的長度約為寰椎總周長的五分之一。前表面為凸狀結構。前弓中央有前結節，其上粘附有頸長肌（英語：Longus colli muscle）和前縱韌帶（英語：Anterior longitudinal ligament）。後表面為凹狀結構，表面分布着圓形或橢圓形的牙槽並與樞椎形成鉸接關節。前弓周圍通過韌帶與枕骨和樞椎相連接[10][11][12]。

後弓[編輯]

後弓連接左右兩個側塊，長度約為寰椎總周長的五分之二。其正中心的棘突為後結節，其上粘附有小後頭直筋（英語：Rectus capitis posterior minor muscle）和項韌帶（英語：Nuchal ligament）。後結節尺寸較小，因而不會限制寰椎和顱骨之間關節的運動[13][11]。

後弓的後部邊緣有圓狀結構，用於連接後寰枕膜。椎動脈和第一頸神經（英語：Suboccipital nerve）後支經過後弓正面一較深的溝，稱為椎動脈溝。椎動脈溝中有一個凹槽，後寰枕膜骨化後，凹槽會變成骨管，其被稱為弓形孔（英語：Arcuate foramen）[13][14][15]。

在椎動脈存在的情況下，上行的椎動脈在會側塊周圍彎曲，穿過椎動脈溝，並進入弓形孔[14]。

後弓的下側有兩個淺槽，後寰樞椎韌帶（英語：Posterior atlantoaxial ligament）附着在其下緣並與樞椎連接[14][16][17]。

側塊[編輯]

側塊是寰椎最厚和最硬的部位，其支撐着頭部的重量。側塊位於寰椎前、後弓的兩側，兩個側塊均有上下關節面[7]。

椎孔[編輯]

寰椎中心的孔洞為其椎孔（英語：Vertebral foramen）。在上關節面的內側邊緣處有一個被寰椎橫韌帶（英語：Posterior atlantoaxial ligament）附着的小結節，該結節使顱骨不均勻地分開穿過寰椎椎孔[18][19][20]。

  • 樞椎的齒狀突結構穿過寰椎椎孔前部[19]。
  • 脊髓和包裹它的脊髓膜穿過寰椎椎孔後部[19]。

寰椎的椎孔半徑遠大於脊髓穿過椎孔所需的半徑，故寰樞椎脫位不會壓迫脊髓[19][21][22]。

成骨作用[編輯]

寰椎的三個骨化中心

寰椎的成骨作用通常從三個骨化中心開始。 其中，後弓的骨化開始於妊娠第7周左右，從左右兩側塊開始向中心骨化。在出生時，後弓中心為軟骨，還未發育完整。直至3－4歲，後弓才骨化完成[15]。

前弓在出生時為軟骨，骨化開始於1歲時，並在6－8歲時形成完整的前弓。大多數情況下，前弓的骨化中心在前結節，但有時並不會出現明確的骨化中心，亦有從左右兩側塊向前結節骨化的情況[23]。

寰椎損傷[編輯]

寰椎椎弓骨折又被稱為傑斐遜骨折（Jefferson fracture），是一種爆裂性骨折。寰椎骨折可造成嚴重後果，甚而導致死亡[8][24][25][26][27]。
